About The Role

We are looking for someone who is passionate about empowering residents to take control of their physical, mental and social wellbeing. In this role you will be supporting us with the delivery of our project in collaboration with Lancashire County Council, looking to reduce contacts into Adult Social Care. You will be committed to reducing health inequalities, as well as have a passion and understanding of the importance of early intervention, taking a holistic approach to identifying needs and areas of support. Working closely alongside the Social Prescribing Team, you will play a vital role in providing the link between services and residents, supporting them to lead happier and healthier lives. 

What you’ll be getting up to
  • You will be supporting residents to improve their mental, physical, and social health, wellbeing and resilience, resulting in a reduced need for accessing Adult Social Care
  • You will prioritise and assess referrals to the service, providing signposting support where needed and working alongside the council’s own Social Prescribing Link Workers.
  • Work with local VCSE organisations, the wider integrated community wellbeing service and other relevant public services to support the efficient functioning of multi-agency work and referrals, helping to build knowledge and understanding of the range of services available in the community, including how and when patients can access them.
  • As well as working alongside existing provision, you will be able to work in partnership with other teams to identify gaps in provision and work to fill those gaps in order for these residents to access the support they need.
Who we’re looking for
  • You will have experience of supporting vulnerable / complex individuals and / or families 
  • You will need to have a good knowledge of local support services and groups that will help ease the transition for the people you will be supporting.
  • Good knowledge and understanding of current issues facing residents of South Ribble and how our service can support with those issues. 
  • Experience of providing holistic support embedded in the practices of health coaching and / or motivational interviewing 
Other requirements
  • This role requires some travel around our boroughs. You should have a valid driving licence or the equivalent mobility.
  • Successful applicants will be subject to a DBS check (applied and paid for by the Council).
